You must complete our Registration Form as well as provide a color copy of your state driver’s license and passport. All licensed clinical professionals must provide a copy of your license.
Make three (3) copies of your passport and birth certificate. Bring two (2) copies of each with you; one (1) will be kept on your person at all times (in your backpack) and one (1) will be kept in the safe in your hotel room. Keep the third copy with a loved one here in the United States.
